Melatonin: Why 0.5 mg Often Beats 10 mg for Falling Asleep

The stakes are practical: extra milligrams don't buy extra sleep — they buy longer exposure, more morning grogginess and, in homes with children, a rising number of accidental ingestions. What the research actually shows

Melatonin is the hormone your pineal gland releases after dark to tell the body that night has begun. Nighttime blood levels peak at roughly 50–200 picograms per milliliter. A 0.3–0.5 mg oral dose reproduces that physiologic range almost exactly, while 3–10 mg tablets push levels well above it — in some dosing studies, more than ten times the natural nighttime peak.

Use has climbed accordingly. An analysis of federal NHANES survey data published in JAMA in 2022 found the share of US adults taking melatonin rose from 0.4% in 1999–2000 to 1.3% in 2017–2018 — modest percentages, but tens of millions of people, most of them self-dosing without a clinician's input.

The key finding is that benefit doesn't scale with dose. In dosing trials conducted at MIT's Clinical Research Center, 0.3 mg of melatonin shortened sleep onset as effectively as larger doses, while 3 mg and 10 mg produced no additional sleepiness — only prolonged, supraphysiologic exposure. The receptors that respond to melatonin, MT1 and MT2, effectively saturate at low concentrations; once they're occupied, extra hormone has nowhere useful to go.

The measured benefits are modest but real. A 2013 meta-analysis of 17 trials found melatonin cut sleep-onset time by about 7 minutes versus placebo, and a 2022 evidence review commissioned by the US Agency for Healthcare Research and Quality reached similar conclusions — benefits measured in minutes, not hours. For a hormone with a benign short-term profile at low doses, that's a meaningful edge over placebo for people whose main complaint is falling asleep.

Why the dose on the label matters

Three practical reasons favor starting low:

  • Next-day fog scales with dose. Melatonin's half-life is short — roughly 30 to 60 minutes — so a physiologic dose largely clears before morning. A 10 mg dose keeps blood levels elevated for hours, which matches the most common complaints at high doses: morning grogginess, vivid dreams and heavy sleep inertia.
  • Labels are unreliable. An analysis of 31 melatonin products sold in Canada found actual content ranged from 83% below to 478% above the label claim, with lot-to-lot variation of up to 465% — and some products contained serotonin. A "5 mg" gummy is, functionally, a dose guess.
  • Child exposures are surging. US poison control centers recorded a 530% rise in pediatric melatonin ingestions between 2012 and 2021 — more than 52,000 calls in 2021 alone — a trend that coincided with the spread of gummies children can mistake for candy.

Our editorial take: the 5–10 mg doses dominating store shelves are calibrated for marketing, not physiology. If a brand only sells high doses, look for a 0.5 mg or 1 mg formulation — several established manufacturers now offer them — rather than splitting a high-dose gummy, which is imprecise.

How to try it

  • Start at 0.5 mg, taken 30–60 minutes before the time you want to be asleep.
  • Hold the same dose and timing for 3–7 nights before judging it. Melatonin is a timing signal, not a knockout drop.
  • If nothing changes after a week, step up to 1 mg. Most adults never need more for sleep onset.
  • For jet lag, take it at the destination's target bedtime on arrival; the NIH Office of Dietary Supplements notes the evidence supports melatonin for jet lag symptoms.
  • Choose brands with third-party verification such as USP or NSF, given the label-accuracy problem above.
  • Talk to a clinician first if you're pregnant, taking anticoagulants, immunosuppressants or diabetes medication, or if insomnia has lasted more than a few months.

The honest counterpoint

The objection

Skeptics have a fair case. The American Academy of Sleep Medicine's 2017 clinical practice guideline recommends against melatonin for chronic insomnia in adults, citing low-quality evidence, small trials and effect sizes that may not reach clinical significance. If the average benefit is five to seven minutes of faster sleep onset, a critic can reasonably ask whether the supplement is worth buying at any dose.

The response

Our response: judge melatonin against what it actually is — a circadian timing signal, not a sedative. The evidence is weakest for generic chronic insomnia and strongest where timing is the underlying problem: jet lag, shift work and delayed sleep phase, in which the body clock runs late. For occasional sleep-onset trouble, a low dose is a proportionate first step before prescription hypnotics, which carry dependence and next-day impairment risks that physiologic-dose melatonin does not. That is an editorial judgment, not medical advice — and for persistent insomnia, cognitive behavioral therapy for insomnia (CBT-I) remains the first-line treatment in every major guideline and should come before any pill, supplement or otherwise.
